Brookhaven Hospital Primary Care Center Celebrates 2nd Anniversary

Brookhaven Memorial Hospital’s Bellport Primary Care Center opened its doors in April, 2014 to address a critical need in providing quality primary care for local residents. The center and its staff also serve as a source of health education for the hospital’s community partners including civic, community and educational groups, to promote wellness and improved health outcomes.
Historically the Bellport community has lacked access to quality health care services and, as a result, residents have had to rely on local emergency rooms to address medical issues or unfortunately, go without care all together. Bellport PCC has been providing these vital health services Monday through Saturday with convenient hours that enable easy access to care for the community. Appointments may be scheduled by calling (631) 227-6600 and walk-ins are always welcome.
The center’s staff includes Board Certified Physicians, a Nurse Practitioner, a Diabetes Wellness Educator/Registered Dietician, and Medical Assistants. Support services, including reception, registration, and insurance verification are provided by Patient Access Associates.
General medical and preventative services provided include:
• Comprehensive physical exams
• School Physicals
• Vaccinations
• Heart disease prevention
• Diabetes and wellness education
• Diet/exercise counseling
• On-site laboratory services
• Cancer screenings
• Depression screenings and referral to treatment,
• Pap smears
• Adolescent medicine
• Linkage to community resources, including Behavioral Health and Health Homes
Plans are also in the works to add a Pediatrician to the staff at Bellport PCC. “We heard from the community that the area is also in need of pediatric health care.” says Dr. Wehbeh, Brookhaven Hospital’s Vice President and Chief Quality Medical Officer. “Having a pediatrician on staff at the Bellport PCC will help us meet those needs as well.”
In December of 2016 the Bellport PCC was selected as a Patient Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA).
The designation is given only to primary care centers that meet the stringent quality standards set forth by the NCQA. A Patient Centered Medical Home focuses on the important role the patient has in their own care. Working together with their Primary Care Physician, the patient develops treatment goals that will not only improve their quality of life, but also emphasize overall wellness and prevention of diseases. The primary physician then coordinates the care with any specialists involved in the treatment, using an integrated care approach to their treatment, making sure to address any emotional or social issues that may be present in their life as well.
Last week Assemblyman Dean Murry took a tour of the facility and presented BPCC and Brookhaven Hospital’s Senior Administration staff with a Legislative Resolution Certificate in commemoration of Community Health Center Week. “It was very impressive to see the impact the Bellport Primary Care facility has had on the community in just two short years.” said Assemblyman Dean Murray. “Through this incredible facility, residents in the community are finally able to receive much needed services that they had not received in the past.”

Margulis, President and CEO of Brookhaven Memorial Hospital Medical Center. “We will continue to make these investments with the goal of being our local community’s health care provider of choice.”
